Statutory Declaration for Application to Change Name (Form 11155E or 11156E)

Your completed Statutory Declaration as part of An Application to Change an Adult's Name or a Child's Name (Form 1155E or 1156E) will need to be commissioned in order to be accepted by the Ontario Government. Dotted Line Notary can help you commission it.
 


What is it?

If you would like to legally change your name as an adult (Form 1155E) or a child's name (Form 1156E) in Ontario, you must complete an application and submit it to Government of Ontario through ServiceOntario.

What do you get if change your name?

If your or your child's name change is approved by the Government of Ontario, a change of name certificate will be issued. It will show both the previous name and new name. The certificate can also be used to to change your name on other personal documents (i.e. driver's licence).

As well, if you were born in Ontario, you will also get a new birth certificate in your new name (but it will not contain parental information but you can submit a Request for Birth Certificate application after).

Requirements

For the adult application, you must be 16 years of age or older and you must have lived in Ontario for the past 12 months. If you are 16 or 17 you will need consent from every person who has legal custody of you. if you are married then you must give your partner / spouse notice that you are changing your name. You must also complete a police record check. The cost is $137 CAD.

For the child application, the child must have lived in Ontario for the past 12 months or since birth if under 1 years old. They must be 17 years old or under. They must not have been married. They must consent to the name change if the child is 12 years of age. There must not be any court orders or separation agreements prohibiting the change of name. Each adult with legal custody of the child must consent to child. Each person who lawfully has access to the child must be provided with written notice. The cost is $137 CAD or $22 (if submitted with a parent's name change application).
