Ontario Student Assistance Program (OSAP) Affidavit

What is it?

If you are a student and are applying for financial aid through the Ontario Student Assistance Program (OSAP) in order to go to college or university, OSAP may require you to provide an affidavit as part of your OSAP application. Common OSAP affidavits include but are not limited to:

  • OSAP Affidavit of Common-Law Status

  • OSAP Affidavit of Sole Support Parent

  • OSAP Affidavit of Student's Foreign Income

  • OSAP Affidavit of No Student Income

  • OSAP Affidavit of Marital Status (Separated or Married)

  • OSAP Affidavit of Indigenous Identity

  • OSAP Affidavit of Spousal Income

  • OSAP Affidavit of Parental Income


Once ready, your OSAP affidavit will need to be commissioned before a commissioner of oaths before beinng provided to OSAP.
